toc missing in the current manual: Gauche-0.9.12-refe.pdf

Hi,

the manual used to have a table of contents, and this was very convient.
The current one (Gauche User's Reference, version 0.9.12),
Gauche-0.9.12-refe.pdf unfortunately does not any more [?]
