RE: TWiki cookbook / licences

I've changed all the converted cookbook pages to use the source code
inclusion mechanism described below, so the speed problem is gone.  I've
documented the new mechanism in IncludingSourceCode.

I've also created an AuthorGuide topic, and a new TOC.  I've added links to
all of this from the Cookbook home page.

I still need to:

* add a link to licensing info to the footer for the twiki page template
* remove the need to use an account to edit topics

I'll probably do that tomorrow.
