Hi Mark,

For Yosemite, here are my settings in Contacts -> Preferences -> Accounts:

Account Information:
Enable this account - Checked
Description - <anything>
User name - <username>
Password - ********
Fetch - Automatically

Server Settings:
Server address - server.domain.com
Server path- /UserDAV/username%40domain.com/
Port - Auto
Use SSL - Checked

I don't use Mac OS X contacts a lot but as far as I can see it works. I can
add contacts in Mac OS and see them in the web GUI and I can delete the
contact in the Web GUI and they disappear from Contacts.

I always had problems with the "Internet Accounts" app in System Preferences
so I wouldn't recommend using that one.
