$MAIL is only used to find the default mbox INBOX.  If you want to use a 
maildir for your INBOX you must explicitly configure it as such.

The first time Cone starts if $HOME/Maildir exists it will be set as the 
default Maildir INBOX.  You must be trying to switch to a maildir 
afterwards, and setting MAIL is not the way to do it.

No.  E-mail headers are, by definition, use folded whitespace to wraparound. 
Cone rewraps all headers to fit within the current screen width.
