Just a matter of interest, how do you apply the patch.
I was thrown in the deep end to get SA-Exim up and running.

Type make sa-exim.h in the sa-exim source directory and copy the file 
where you copied local_scan.c

I forgot to include that in the documentation for that release, sorry
(honestly, I thought pretty much everyone would patch exim with the
dlopen_localscan patch
