I do

patch -p1 < ../path/to/localscan_dlopen_exim_4.20_or_better.patch
make && make install

Is that what you did?

I am not sure about the cause, but you need to have patched correctly
and besides, you need to have edited the Makefile (for sa-exim) to point
to the correct path for exim src.
