Hmm, that makes me feel like an idiot.

You mentioned that you have done updates on php and mysql recently, how 
are the modules handled on php on your system?  On mine they are 
separate binary libs which can be dynamically loaded.

Also, try changing this line in php.ini from
error_reporting  =  E_ALL
to
error_reporting  =  E_ALL & ~E_NOTICE

This should eliminate the notice errors which aren't really important at 
this stage.
