MacOS X supports two case-sensitive filesystems; UFS and 
HFS+-case-sensitive (a special variety of HFS+ that is case 
sensitive). Perhaps the easiest method is to simply start using one of 
those filesystems instead.

However, I wouldn't recommend using qmail on MacOS X. It's got some 
bugs that are exposed by qmail, particularly under load. This can 
cause qmail to hang.
