Probably neither.

I've found two common cases with vanilla ezmlm.  At least one, and possibly
both, apply equally to -idx:

1) User signed up with one address, but then had mail forwarded from that
account to another.  He's forgotten this, and thinks he's signed up with the
second address.  When he sends mail from that second address to
<list-unsubscribe@...>, ezmlm will return a message that *looks* like an
unsubscription confirmation.  When he then continues to receive list mail, he
gets upset.  You'd need to see a complete message header to confirm/help.

2) User subscribed with their address in mixed-case.  When they try to
unsubscribe, they've got their address in a different case (i.e., subscribe
with <JOHN@...> or <John@...> and trying to unsubscribe from <john@...>.
Plays out as above.  I seem to recall Bruce changed -idx to collapse case at
some point, but I could be wrong or your installation could predate that
version.
