Establishing A Form Form EZMLM Subscriptions On A Web Page

I'm a new member of the ezmlm mailing list. I'm setting up a listserv  
using ezmlm on a site I'm developing, and we want to put a form on a  
page to collect the e-mail address for subscribing. I haven't been  
able to figure out how to take the contents of the form field (the e- 
mail address) and submit it to the
ezmlm list I've established. Can someone point me to a tutorial on how  
I can pull this off?
