I forgot to mention one of the changes.  Since DJB has placed ezmlm-0.53
into the public domain, this is the first version of ezmlm-idx that no
longer requires patching against ezmlm.  The source tarball is all that
you need (well, the source and a compiler, and qmail, and... :)
