In the OP's case, turning delete_after on now -- for argument's sake, say it's set to 30 days - then starting with a clean slate will result in getmail retrieving all messages in the account. If the OP keeps running getmail, then these messages will start getting deleted 30 days after they are retrieved. In the meantime, new messages received will be retrieved, but will be deleted correspondingly later. There's no magic way to say "delete older messages now and newer messages on a schedule", but since the difference between the above and this vanishes over time - after X days, there's no difference at all - there's no need for it.
