I looked again, and it appears I was wrong. getmail-gmail-xoauth-tokens DOES do the refresh. So maybe something went wrong during the refresh (network problems?) or maybe you had a long break/vacation during which no refresh did take place, and the refresh token expired. Normally the refresh also gives a new refresh token, with a new expiration time. However, the expiration time of the regresh token is not recorded, only the expiration time of the access token.
