the server needs to keep a timeout.  i think we currently agree that
the "download URL" is very hard to guess, so the server knows
automatically who has fetched something:  only the receiver knows
that magic word.  to reclaim storage, it needs the timeout.

with a publish/subscribe system, delivery status is entirely the
receiving MUAs business.
