"autocreate_quota: -1" disables autocreate; you should use 0 or higher.

Wild guess; are you removing your test mailbox each time you try? The 
autocreate functionality is only executed on first post or login; if the 
mailbox existed prior to adding the autocreate_inbox_folders 
configuration option, it wouldn't be triggered on subsequent logins.
