You can use notifications by putting in imapd.conf, e.g.


   event_notifier: external
   notifysocket: /var/run/cyrus/socket/notify
   notify_external: /usr/local/bin/trainhamspam


Then you have to write a program (here /usr/local/bin/trainhamspam) to 
manage the notifications

I have one such program here https://github.com/olivluca/trainhamspam 
but of course it only works with my setup.
