Not sure if this will help but I use the following:
suffix      "dc=mydomain,dc=com"
rootdn      "cn=Manager,dc=mydomain,dc=com"
rootpw      {SSHA}mypQ1AjN4Bvs/Test123uHj5tpysi8r3

No, it is not the real hash, but I think you get the idea and it WORKSFORME.

I am using the same here. Are you sure /etc/nsswitch is set correctly??

Neither does courier. For postfix you add a trailing / to the mailbox command.
