Oh, I DO reject during SMTP...97%-98% of connection attempts are 
rejected by my servers.  I have built a rather elaborate but effective 
process that adds entire netblocks to be rejected upon receipt of spam 
from specified countries.  If I ever get a customer who wants to do 
business in China, or Russia, or Korea, I'm going to have a problem.  
Anything that gets past the by-country blocks is then scanned, with 
about a 60% rejection rate, but I'm only scanning 1%-3% of the attempted 
connection count.  Only stuff that gets past all that gets bounced...AND 
reported to the originating ISP.
