Extremely Odd Non-Delivery

My Mj2 server is working well... except...

One moderated list in one domain is not delivering its messages.  
Instead, the approved messages show up in the archive multiple times, 
and never delivered.  No post-injection MTA log entries are found.

Advice on debugging?
