I have seen this in the past when a mail loop was established by 
software that rejected a message by returning it unaltered to its point 
of origin.  If that's what is happening, what you should do next is find 
out which of your subscribers' e-mail addresses is responsible for the 
loop.  The "Received" headers of the message in the attachment may 
provide some helpful evidence.

The access rules are only applied when a message first arrives, and have 
no effect upon a message that is approved by the moderators.
