You should do a virus scan on the MTA side, not neomail.

The starting point to inject an ad into every sent mail is in neomail.pl
something above
      close(SENDMAIL) or senderror();

Yes, this should be possible (if your programmer knows how *g*)

I would do this with a database backend, which stores the ads, the
number of emails sent with every ad, so you have control, how often
every ad was sent. For neomail i would develop a "plugin" or better
a sub , which retrieves the ad from the database, updates the database
(how often the ad was sent and when was the last transmission of the ad)
