neomail was written at a time, where perl 5.8.x was not the production
perl. That's the reason for the need of the "insecure ..." patch you
already installed.
The perl MD5 module:

My intention with the advice to install the modules is, to make
sure, that both modules are up to date.

Consult your apache log and neomail.conf to see, where neomail expects
the images and where apache tries to serve the images.
Now edit neomail.conf (and copy the images to the right location)
