Yes. Neomail doesn't care about the DocumentRoot. It only gives
a reference to the apache "virtual" root.
This means:
If you have in image_url -> /images
the physical path to an image is: 
DocumentRoot/images/left.gif
The "http path" is:
http://server.com/images/left.gif

Again, only the apache error log will tell you (us) what's happening.
If it's a permission problem or a "file not found" problem is to be seen
in the error log.

If it's a standard apache installation, you do not need to
restart apache or neomail, as neomail is everytime interpreted new
if you call it. Changes in neomail.conf and neomail.pl are visible
in real time. If you change settings in the apache config (httpd.conf)
you have to SIG HUP or restart apache (or httpdctrl graceful).

That's not problematic, as neomail.pl parses neomail.conf and gets
values from neomail.conf in runtime.
