Apparently, there is a (actually more like 5 subtly different) non-standard xterm extension that, when activated by sending special sequences to the xterm, causes xterm to report all mouse (including wheel) events to the application. I'm not sure this can be used safely and/or portably since far from everything that calls itself an xterm is in fact an xterm... but vim is widely deployed so perhaps a look in the vim sources would show a good way to do this.
