If it was some other cause - like hardware issues - it wouldn't always succeed with one domain and fail with the other. Finding out which patch or combination of patches causes the bug will not be simple. You may wish to try reverting to vanilla qmail and add the patches you need back in one at a time -- i.e., don't use a big combined patch, just patch in the features you need. Add a patch, run the test case - if it doesn't crash, move on to the next patch, etc. Once it starts crashing, you've found it.
