Well, you may search the net for qmail-ldap-1.03-20030401-badhelo.patch
for example using a mailinglist archive like http://marc.info/
where you would find
http://marc.info/?l=qmail-ldap&m=105104069700968&w=2
I haven't looked if it also checks for empty helo/ehlo but maybe it fits
your needs.

We used this patch as a base for our own changes as we integrated the
qmail-1.03 based badhelo regex check.
