See contrib/qscan-spam-to-users.pl

You can run it hourly/whatever, and it will move all mail out of
/var/spool/qscan/quarantine/spam into a "real" account Maildir structure
(user: qspam). It is oriented around Courier-IMAP, and will mean that
you can login via IMAP into that account and see all the SPAM being sent
to your users - separated into individual folders per recipient. It also
self-cleans to ensure the Maildir doesn't grow indefinitely.

There's a couple of buglets in that version (fixed in the next release).
I've just put up the fixed version for one-off downloading until the
next release comes out: http://qmail-scanner.sf.net/qscan-spam-to-users.pl
