Hi Maciej

There are some possibles causes for a message not being tagged

- the timeout of spamc client, you can change it to a higher value 
editing qmail-scanner-queue.pl and searching for the variable 'my 
$spamc_options' at the start of the file (in 2.01st search for 'my 
$spamc_binary='/usr/bin/spamc -t 30';')

- the message is bigger than 250 k then it is skipped, then you would 
find something in qmail-queue.log

- using verbose_spamassassin (bad thing) and customizing the headers 
it is possible to break the check of qmail-scanner...
