this /should/ be enough to tip you off...

what does the ownership and perms look like for
"/var/spool/qmailscan/qmail-scanner-queue-version.db"?

perhaps they should match
"/var/spool/qmailscan/qmail-scanner-queue-version.txt"...

my gentoo boxes look like:
-rw-r-----  1 qscand qscand 12288 Jun 27 16:43 quarantine-attachments.db
