If seems to me that you have some problems with your qmail installation,
maybe by the patch. The files in the directories /var/spool/qmailscan/tmp
and ../working are from old mails and they will lay there until you purge
it (always with qmail off to avoid deleting a still processing mail). The
problem with qmail-queue and dying parent process come from elsewhere.

Try increasing softlimit.
