The first time welcome screen never showed up, nor did the > toolbar. The add-in itself wasn't loaded. So this means that something is wrong with the source install environment. Unfortunately I can't guess what that is - but what we need to aim for is identical functionality from the source and binary versions before attempting to modify the source version.
