Not unless they already know my account number with the brokerage, my
date of birth, my wife's date of birth, employer's name, etc. etc.
etc., which are on the form.

That, and it states that I should mail any corrections to the same
address, or fax to the same number, that's posted on the brokerage's
website.

And the letter is dated "April 28", and not the 1st.
