Maybe an idea here.... (a little sneaky, but so what)

If you get into the program and they want to keep you for more than 6 
months, maybe they can release you on "Home Leave" for about a week or two 
at the 6 month mark so you can re-establish residence and then re-admit you 
for another 6 months if necessary. That worked for my mother in Calif when 
she had to go in for re-hab.

As for the visiting psych nurse, keep good records of what he does wrong. 
Also maybe write a formal letter to the agency detailing the problems. Even 
if they don't do anything, you have proof that you are on top of things and 
are trying to get help.
