TMDA-cgi-0.16.2 is released

Another fabulous candidate in the 0.16 "Sulphur" bugfix series.  This
includes fixes for:

- Previous 0.16 releases did not delete '.svn' hidden directories,
  which broke the auto-install feature.
- Includes fix for vmailmgr stub to allow users with '.' in the names.
- Fixes the "Whitelist releases a message twice" issue.
- Installed template for confirm_accept.txt is now correct.
