Could it be enhanced further to distinguish between an authenticated
user's manual release of the message using tmda-cgi, and an
"auto-release" triggered by a tmda-cgi URL in a challenge email?

Maybe augment tmda-pending and tmda-rfilter to do the same/similar
thing, so there's always a header indicating why the message was
released, and the way it was released?
