A couple questions to narrow this down:

Are you using virtual or system users.

Does tmda-ofmipd run as root or some other fixed user ID.

Does tmda-ofmipd call tmda-inject directly, or is there some wrapper 
script that you added in between to do any other processing?

That should at least narrow down the code path a tiny bit.
