pending messages NOT released

Hi

I'm running Courier 0.57.1 & TMDA 1.1.9
my problem is that my pending folder does not release messages

I receive the confirmation request and subsequent "your message
has been released" emails.  The address is even added to the whitelist
and no longer requires a confirmation on the second email.

is there something I"m missing?  if anyone could just nudge me in the
right direction I'd greatly appreciate it.
