Mail pending, no confirmation sent out

I'm sure this has been covered but I can't figure out why it's  
happening.

I've set up tmda following the instruction. I had some help with cgi  
stuff.
If I send an email from an outside account I see the section with...

in my incoming log. I also see the email in my pending directory. But  
that seems to be
about all that happens. I don't  get a confirmation email? I don't  
see anything in the debug log,
or anywhere else that I can see.

This is an OS X server
