I don't know that I can help directly, but I'd recommend:

    1. Upgrading to a more recent version. There were a few
       pending-queue issues that were addressed in past releases.

    2. Look at the verbose logs for your delivery mechanism. For
       example, if you're using procmail, then your delivery line should
       look something like '|procmail ~/.procmail-tmda' and you should
       set "VERBOSE=yes" in your .procmail-tmda resource file.

My guess is that there's something wrong with your procmail recipes, or
your tmda delivery recipes if you've defined any in
.tmda/filters/incoming.
