Most likely, something isn't passing the address "extension" to TMDA, so 
it can't tell that the confirmation message is a confirmation message.

If that doesn't help, you'll need to go through your qmail, maildrop, 
fetchmail, and TMDA log files and trace where the process goes wrong.
