action_missing_pending problem

After years of tmda work fine, I have one user only -me-, that is getting
the error action_missing_pending

When someone new sends and email, they get the please confirm email,
when the user confirms they get an error message;

On issuing the tmda-pending command from the shell - it shows no
emails pending - but the emails actually do exist.
I have checked permissions, and ownership of directories and files.

Any pointers would be appreciated.
