I don't know anything about this configuration, but doesn't the description of this script (http://tmda.sourceforge.net/cgi-bin/moin.cgi/TmdaPostfixPolicy) explicitly say confirming isn't supported? It kind of seems like confirming should still be supported as a delivery step (e.g., if this is combined with the more traditional ~/.forward filtering), but perhaps there's some reason this doesn't work.
