But I do think that the situation you have, where the printed version of a message is vastly different from the displayed one, is a very specialised use case. If you look around, any application I found where I press print, I get something that in displayed information by far and large mirrors that which was on the screen. That is something that SquirrelMail users expect not differently. Sure, I'm more than willing to see how we can make SquirrelMail flexible enough to implement large parts of Melinda. I even think that the things you say can be done in the current interface, although a bit different from how you probably implemented them previously. So I'm not a priori convinced that there's a need to keep the complete old printer friendly system in parallel. But even then, there will be a point when a plugin's requirements will get so specialised that it makes sense to move the code into the domain of that plugin. Upto now I don't see a wide need for being able to have the printed version of an email be sweepingly different from the screen version. If only your plugin requires that, it's a thing that is exactly the thing that makes your plugin different from the rest of SquirrelMail and not a problem per se to have that code in the domain of your plugin.
