Work has begun on Dick's maia fork -
https://github.com/einheit/maia_mailguard_1.05

For now, other than configtest.php. all the Maia web interface
functionality is broken, and will remain so, until all the PEAR::MDB2 based
code has been replaced with equivalent PDO-based code.

At this point I'm simply starting with config.php and maia_db.php, making
updates, then hitting the maia landing page and looking at the httpd error
log. Lather, rinse, repeat.

At the point where any of the files seems to be approximately correct, I'll
submit a merge request with that updated file. That doesn't mean it's 100%,
but that it's at least something to work with.

Interesting times ahead.
