Hi Bubba,
You can checkout the repo and either use git to apply the patch ("cd /usr/ports && git apply /path/to/patch" if you're using git for the port tree) or you can put put the maia-mailguard directory into the /usr/ports/security port directory.
Once the patch or directory are in place, you would just install as a normal port ("make all install clean" or whatever tool you choose to use.)
