She probably thought she was being clever and literate, as it was originally
used Edward Bulwer-Lytton in 1830.

I think in her context she is using it in place of flyover country, or
heartland.  It just emphasizes that she see the coast regions in a different
light than the heartland, which is a given.  But she was apparently completely
ignorant to the real meaning of the phrase.

I am surprised that she didn’t understand the negative connotations.

Um... no room, then, for the possibility that Couric simply had a
different understanding (right, wrong, or otherwise) of what the term
means?
