Until about 2004 in Ontario we didn't pay anything beyond our income
taxes, then they added a new "premium" that is lumped in with our
income taxes.  From wikipedia:

"The Ontario Health Premium is an additional amount charged on an
individual's income tax that ranges from $300 for people with $20,000
of taxable income to $900 for high income earners. Individuals with
less than $20,000 in taxable income are exempt."

It hasn't gone up at all since then, but it was very controversial as
the Premier was freshly elected and had promised not to raise taxes.

Still, people either pay fees for or pay for health insurance for:
dental
optical
orthotics
prescriptions
massage therapy
chiropractors
private rooms at hospitals for after giving birth
some treatments and medications that haven't been approved yet
other stuff I can't think of at the moment

I'm content with the system, and they have made progress on wait
times.  The key challenge is to eliminate waste and excess bureaucracy
to keep costs under control.
