 FWIW, Ableton (software DAW on Windows and Mac) does have that feature -
the "Harmony to Midi" function will generate polyphonic Midi from any Audio
source.
However:
  - It's far from realtime - it is very much an offline process. Consumes
alot of CPU, and takes a long time at that.
  - It is certainly not infallible - its accuracy depends on the overtones
in the audio stream from whatever instrument(s) were used, the attack of
the notes, any modulation, etc. etc. So typically, you will spend time
deleting "false triggers" in the generated midi - and I notice that any
low-frequency notes detected are often a semitone off. It does however
generate midi that matches up in the timeline with the source audio, and it
can produce interesting chords from the source audio...

Then there's Celemony Melodyne - again, software. I don't own it, but
afaik, it does do the audio->midi thing.

There may well be iPad apps or similar that purport to doing similar. You
might want to investigate that if its an option.
