Do you use Win 9x? There is a special option in BNR to check for 
free space in a drive root:

Changes in BNR2 Beta 0.11.5
- Replaced the one period advanced option with an option to 
perform the free space checks on the root directory of the target 
drive instead of the actual download directory to get around bugs 
in the GetDiskFreeSpaceEx function in Windows 95, 98, 98SE and 
ME. This option is on by default when creating a new install 
under an older Win9x-class version of Windows (95, 98, 98SE, ME), 
and off by default in an NT-class version of Windows (NT, 2000, XP).

Can it be that the "use part of subject line as directory name" 
(or whatever is it's exact name) option is on and BNR tries to 
create directory with invalid name, since the replacement invalid 
and/or trim characters are switched off or missing some character?

I remember having similar problems because of a *space* character 
not being included in the list of chars to trim from 
beginning/end of file- and directory names.
