You're probably missing a schema. I always have this error when I try to say
"This user can login from all hosts" (or some such) when creating new users.
Try unselecting that checkbox (second screen, I think) and don't fill in any of
the fields on that screen and continue on. It should then create fine (though
yours may be from some other missing schema).
